School Clinic

This year, we are partnering with Children’s Hospital to oversee our school clinic and clinic aids. Children’s Hospital Colorado School Health Program was established in 1984 and is continually evolving to meet the needs of children in various settings along the Front Range area. Their services are provided based on processes that align with national, state and local standards, the Colorado Nurse Practice Act and the Children’s Hospital policies and procedures.

All medications, including over-the-counter medications (tylenol, ibuprofen, benadryl, tums, etc…), require a doctor’s signature before acceptance into the clinic or administration to a child.

Please print and complete the forms below if your child has Allergies, Asthma or will need to take any Medication during school hours. These completed forms will need to accompany all medications before intake is permitted.

MPCS is an asbestos-free school. Copies of our Asbestos Management Plan are located in both the main and middle school offices for public viewing.
